A 2024 analysis in the Journal of Ethnopharmacology pooled 23 trials from 7 countries — 2,175 adults with knee joint pain in total — comparing six different approaches: curcumin alone, curcumin combined with a standard anti-inflammatory drug, curcumin combined with a joint-support supplement, the anti-inflammatory drug alone, and placebo. Compared with placebo, curcumin produced a meaningfully larger drop in both a standard 0-10 pain rating and a composite pain-and-function questionnaire used in joint research. People taking curcumin, whether alone or alongside the drug, also needed less rescue pain medication than those on placebo, and reported fewer side effects than the group taking the anti-inflammatory drug by itself.
A separate 2025 review in BMC Complementary Medicine and Therapies pooled 17 randomized trials (searched across four major research databases through August 2024) and broke the results down by the type of curcumin product used. Every formulation tested reduced the pain subscore of that same joint-function questionnaire compared with placebo — but the size of the effect varied a lot by formulation. A standard, unmodified curcumin extract produced a moderate reduction; an absorption-enhanced version produced a smaller-looking number on paper but one that still cleared the threshold researchers consider a noticeable, real-world difference to the person reporting it; and curcumin paired with a standard anti-inflammatory drug produced the largest reduction of any combination tested.
Across both analyses, the direction of the result is consistent: curcumin beat placebo on pain and stiffness in every pooled comparison, the size of the benefit was moderate rather than dramatic, and it took several weeks of daily use to show up — not a one-time dose.
Plain curcumin is notoriously hard for the body to absorb — it's broken down and cleared quickly, so blood levels can stay too low to do much. That's why almost every trial behind these results used a modified extract, not culinary turmeric powder: some paired curcumin with piperine (a black pepper compound that slows that breakdown), others used a fat- or phospholipid-based delivery form designed to raise absorption. The 2025 review's formulation breakdown makes this concrete — the products built specifically to improve absorption behaved differently, not just better, than a plain curcumin extract. Sprinkling turmeric on food is a fine culinary habit, but it isn't the thing any of these trials actually tested.
A few caveats are worth sitting with before drawing a firm conclusion. Most individual trials feeding these pooled analyses were still fairly small and ran for a matter of weeks, not months or years, so they speak to short-term comfort rather than any long-term change in joint health. The people enrolled were mostly adults with diagnosed knee joint pain, so the results may not carry over cleanly to joint discomfort with a different cause or location. Pain scales in this kind of research are self-reported, which leaves room for a real placebo response to blend into the measured effect — a factor these pooled analyses try to account for by comparing against placebo groups directly, but can't fully erase. And a meaningful share of the underlying trials were funded by supplement manufacturers, which doesn't invalidate the data but is a reason to weight it alongside, not instead of, independently funded research.
If you're weighing a curcumin supplement for everyday joint comfort, the trial data points to a few concrete things worth checking on a label: an absorption-enhancing ingredient such as piperine, or a stated bioavailability-enhanced formulation, since that's what most of the tested products actually were. Expect a gradual change over several weeks of consistent use rather than immediate relief. Curcumin can have a mild blood-thinning effect and can interact with anticoagulant medication, so it's worth a conversation with a healthcare provider first if you're on blood thinners, have a bleeding disorder, or take regular anti-inflammatory medication.
